Non-Contrast-Enhanced MR Arteriography of Potential Living-Related Liver Donor: Using Contrast Enhanced CT Arteriography as Standard Reference.

Contrast-enhanced computed tomography angiography (CTA) and magnetic resonance angiography (MRA) are the primary modalities to assess donors' vessels before transplant surgery. Radiation and contrast medium are potentially harmful to donors. To compare the image quality and visualization scores of hepatic arteries on CTA and balanced steady-state free-precession (bSSFP) non-contrast-enhanced MRA (NC-MRA), and to evaluate if bSSFP NC-MRA can potentially be a substitute for CTA. Prospective. Fifty-six consecutive potential living-related liver donors (30.9 ± 8.4 years; 31 men). 1.5T; four bSSFP NC-MRA sequences: respiratory-triggered (Inhance inflow inversion recovery [IFIR]) and three breath-hold (BH); and CTA. The artery-to-liver contrast (Ca-l) was quantified. Three radiologists independently assigned visualization scores using a four-point scale to potential origins, segments, and branches of the hepatic arteries, determined the anatomical variants based on Hiatt's classification, and assessed the image quality of NC-MRA sequences. Fleiss' kappa to evaluate the readers' agreement. Repeat measured ANOVA or Friedman test to compare Ca-l of each NC-MRA. Friedman test to compare overall image quality and visualization scores; post hoc analysis using Wilcoxon signed-rank test. P-value <0.05 was considered statistically significant. Inhance IFIR Ca-l was significantly higher than all BH bSSFP Ca-l (0.56 [0.45-0.64] vs. 0.37 [0.29-0.47] to 0.41 [0.23-0.51]). Overall image quality score of BH bSSFP TI1200 was significantly higher than other NC-MRA (4 [4-4] vs. 4 [3 to 4-4]). The median visualization scores of almost all arteries on CTA were significantly higher than on NC-MRA (4 [3 to 4-4] vs. 1 [1-2] to 4 [4-4]). The median visualization scores were all 4 [4-4 ] on Inhance IFIR with >92.3% observed scores ≥3, except the segment 4 branch (3 [1-4], 53.6%). The identification rates of arterial variants were 92.9%-97% on Inhance IFIR. Although CTA is superior to the NC-MRA, all NC-MRA depict the donor arterial anatomy well. Inhance IFIR can potentially be an alternative image modality for CTA to evaluate the arterial variants of living donors. 3 TECHNICAL EFFICACY: Stage 2.

Alterations in cerebral distal vascular features and effect on cognition in a high cardiovascular risk population: A prospective longitudinal study

BackgroundAlterations in cerebral vasculature are instrumental in affecting cognition. Current studies mainly focus on proximal large arteries and small vessels, while disregarding morphology and blood flow of the arteries between them (medium-to-large arteries). MethodsIn this prospective study, two types of non-contrast enhanced magnetic resonance angiography (NCE-MRA) techniques, simultaneous non-contrast angiography and intraplaque hemorrhage (SNAP) and 3D Time-of-flight (TOF), were used to measure vascular morphologic features in medium-to-large intracranial arteries. Grey matter (GM) tissue level perfusion was assessed with arterial spin labeling (ASL) MRI. Twenty-seven subjects at high cardiovascular risk underwent baseline and 12-month follow-up MRI to compare the relationship between morphological features measured by NCE MRA, GM CBF by ASL MRI, and cognitive function measured by the Montreal Cognitive Assessment (MoCA). ResultsChanges in both global medium-to-large arteries and posterior cerebral (PCA) distal artery length and branch numbers, measured on SNAP MRA, were significantly associated with alterations in MoCA scores (P < 0.01), after adjusting for clinical confounding factors, total brain volume, and total white matter lesion (WML) volume. There were no associations between MoCA scores and vascular features on TOF MRA or ASL GM CBF. ConclusionsAlterations in vascular features of distal medium-to-large arteries may be more sensitive for detecting potential changes in cognition than cerebral blood flow alterations at the parenchymal level captured by perfusion ASL. Hemodynamic information from distal medium-to-large arteries provides an additional tool to advance understanding of the vascular contributions to cognitive function.

Type 2 endoleak embolization and utilization of non-contrast-enhanced magnetic resonance angiography (NCE-MRA) as a non-invasive imaging follow-up method.

Endovascular aneurysm repair (EVAR) is a minimally invasive technique widely used in abdominal aortic aneurysm treatment. The most common complications after EVAR remain endoleaks, type 2 endoleak (T2EL) being the most prevalent. T2EL detection and surveillance require imaging techniques such as computed tomography angiography (CTA), contrast-enhanced magnetic resonance angiography (CE-MRA), Doppler ultrasound, or digital subtraction angiography (DSA). However, these modalities are associated with numerous limitations, including exposure to ionizing radiation, contrast media administration, or operator dependency, as in the case of ultrasonography. A non-contrast-enhanced MRA could be a substitute non-invasive method for endoleak monitoring. Our case report describes an 83-year-old female patient with type 2 endoleak and enlarging aneurysm sac detected on a CT scan. Despite the enormous aneurysm size, the patient underwent endovascular treatment owing to multiple comorbidities. Due to challenging feeding vessel anatomy, catheterization of the aneurysmal sac was impossible. Attempted polymerization of the aneurysmal sac with Glubran-2 partially sealed the sac and obliterated the feeders' inflow. Unfortunately, a non-targeted embolization resulted in the loss of patency of the right feeder and adjacent communicating branch. The patient underwent follow-up imaging that included non-contrast-enhanced as well as contrast-enhanced MRA. The examination revealed the presence of a small residual endoleak, a freshly formed thrombus, and areas of old thrombi. The NCE-MR appeared to be a valuable tool in endoleak detection and provided a detailed clot morphology.

Associations of intracranial artery length and branch number on non-contrast enhanced MRA with cognitive impairment in individuals with carotid atherosclerosis.

Developing novel risk markers for vascular contributions to cognitive impairment and dementia is important. This study aimed to extract total length, branch number and average tortuosity of intracranial distal arteries (A2, M2, P2 and more distal) from non-contrast enhanced magnetic resonance angiography (NCE-MRA) images, and explore their associations with global cognition. In 29 subjects (aged 40–90 years) with carotid atherosclerotic disease, the 3 intracranial vascular features on two NCE-MRA techniques (i.e. time of flight, TOF and simultaneous non-contrast angiography and intraplaque hemorrhage, SNAP) were extracted using a custom-developed software named iCafe. Arterial spin labeling (ASL) and phase contrast (PC) cerebral blood flow (CBF) were measured as references. Linear regression was performed to study their associations with global cognition, measured with the Montreal Cognitive Assessment (MoCA). Intracranial artery length and number of branches on NCE-MRA, ASL CBF and PC CBF were found to be positively associated with MoCA scores (P < 0.01). The associations remained significant for artery length and number of branches on NCE-MRA after adjusting for clinical covariates and white matter hyperintensity volume. Further adjustment of confounding factors of ASL CBF or PC CBF did not abolish the significant association for artery length and number of branches on TOF. Our findings suggest that intracranial vascular features, including artery length and number of branches, on NCE-MRA may be useful markers of cerebrovascular health and provide added information over conventional brain blood flow measurements in individuals with cognitive impairment.

Comparison of 1.5 T and 3 T non-contrast-enhanced MR angiography for visualization of uterine and ovarian arteries before uterine artery embolization.

This study aimed to compare the performance of non-contrast-enhanced magnetic resonance angiography (NCE-MRA) at 1.5 T and 3 T for the visualization of uterine and ovarian arteries (OAs) before uterine artery embolization (UAE). Preprocedural pelvic NCE-MRA images of 85 symptomatic patients undergoing UAE for the treatment of uterine leiomyomas were reviewed by two specialists in pelvic MRI. Left and right uterine arteries (UAs) were judged separately and scored on a 5-point scale. Score 5 was the highest, in which the UA could be visualized inside the musculature, forming a peritumoral plexus. Score 1 was the lowest, where visualization was limited to the descending segment. The detection of enlarged OAs was also compared. The Mann-Whitney U and Fisher exact tests were used for statistical analysis. p < 0.05 was considered to be statistically significant. Of the 170 UAs, 110 were classified at 1.5 T and 60 were classified at 3 T. Median (interquartile range [IQR]) score was 3 (IQR: 2-4) for visualization at 1.5 T vs 5 (IQR: 4-5) for 3 T. The scores for UA visualization were significantly higher at 3 T (p < 0.05). For enlarged OAs, NCE-MRA at 1.5 T and 3 T visualized 7 and 5 enlarged OAs, respectively; there was no significant difference between the two field strengths (p = 0.36). NCE-MRA performed at 3 T can visualize UAs over a greater range than at 1.5 T. No difference was found regarding the detection of enlarged OAs. • Preprocedural MRA can provide interventional radiologists with valuable information, including the origin and course of the uterine arteries and the existence of collateral feeders to the tumor. • This study demonstrates the superiority of non-contrast-enhanced MRA performed at 3 T over that performed at 1.5 T in the visualization of the uterine arteries in patients undergoing uterine artery embolization for the treatment of uterine leiomyomas. • Non-contrast-enhanced MRA is a useful imaging modality for patients with symptomatic leiomyoma undergoing uterine artery embolization in whom contrast administration is unfeasible. If available, it is preferable to perform the examination with a 3 T MR unit rather than a 1.5 T MR unit.

Evaluation of Renal Allograft Vasculature Using Non-contrast 3D Inversion Recovery Balanced Steady-state Free Precession MRA and 2D Quiescent-interval Slice-selective MRA.

Background and objectives:Renal transplant patients often require periodic imaging to evaluate the transplant vessel anastomosis for potential vascular complications. The use of non-contrast enhanced magnetic resonance angiography (NCE-MRA) techniques is encouraged in these patients because they are at increased risk of nephrogenic systemic fibrosis (NSF) due to their renal insufficiency. This study aimed to evaluate the performance of two NCE-MRA techniques (three-dimensional [3D] balanced steady-state free precession [bSSFP] with inversion recovery and quiescent-interval slice-selective [QISS]) for the evaluation of renal allograft vasculature in patients with clinical suspicion, or Doppler ultrasound, or both of arterial anastomotic stenosis.Methods:A total of 43 patients were included in this retrospective study. Two radiologists independently scored the images from 3D bSSFP and QISS MRA sequences for image quality and confidence in anastomosis interpretation, and the degree of stenosis at the arterial anastomosis. Correlations with digital subtraction angiography (DSA) were carried out when available. In addition, inter-rater agreement was calculated.Results:In total, 43 patients underwent QISS and 3D bSSFP MRA. For QISS, all cases were adequate for evaluation. For 3D SSFP, 86% of cases were adequate for evaluation. There was a good-to-excellent inter-rater agreement for all scores and an excellent correlation between NCE-MRA and DSA results when available (12 patients).Conclusions:QISS and 3D SSFP showed good inter-rater agreement for image quality and stenosis grade, with more cases being of adequate image quality that used QISS. Further study is required; however, NCE-MRA shows potential as a risk-free alternative to CTA and contrast-enhanced MRA (CE-MRA) for the evaluation of arterial anastomoses in renal transplant patients.

MRI-Based 3-Dimensional Visualization Workflow for the Preoperative Planning of Nephron-Sparing Surgery in Wilms’ Tumor Surgery: A Pilot Study

Purpose. Due to the size and localization of Wilms’ tumor (WT), nephron-sparing surgery (NSS) is only possible in a limited number of cases. When NSS is considered, the surgeon preoperatively requires a thorough understanding of the patient-specific anatomy to prevent positive surgical margins and other complications. Through a collaboration between the radiology and pediatric surgery departments and 3D imaging specialists, a 3D visualization workflow was developed to improve preoperative planning of NSS for WT patients. Methods. The 3D visualization workflow combines a MRA sequence, a segmentation protocol, and augmented reality (AR) visualization, additional to in-house 3D printing. A noncontrast-enhanced MRA scan was added to the MRI protocol. MRI sequences were segmented with a segmentation protocol in an open-source software package. The resulting 3D models were visualized in AR with a HoloLens and 3D print. Results. In a pilot study, five WT patients eligible for NSS were preoperatively planned through the 3D visualization workflow. AR visualization software was fast and free to use and allowed adequate handling of the 3D holograms. The 3D printed models were considered convenient and practical for intraoperative orientation. The patient-friendly, fast, and low-cost 3D visualization workflow was easily implemented and appeared to be valuable for the preparation of NSS. Conclusion. This pilot study demonstrates how a strong collaboration between the pediatric surgery and radiology departments and 3D imaging specialists will help to shape the future of pediatric oncological surgery. This 3D visualization workflow aims to prepare pediatric oncological surgeons for nephron-sparing surgery in patients with Wilms’ tumors.

Clinical Value of Noncontrast-Enhanced Radial Quiescent-Interval Slice-Selective (QISS) Magnetic Resonance Angiography for the Diagnosis of Acute Pulmonary Embolism Compared to Contrast-Enhanced Computed Tomography and Cartesian Balanced Steady-State Free Precession.

Free-breathing noncontrast-enhanced (non-CE) magnetic resonance angiography (MRA) techniques are of considerable interest for the diagnosis of acute pulmonary embolism (APE), due to the possibility for repeated examinations, avoidance of side effects from iodine-based contrast agents, and the absence of ionizing radiation exposure as compared to CE-computed tomographic angiography (CTA). To analyze the clinical performance of free-breathing and electrocardiogram (ECG)-gated radial quiescent-interval slice-selective (QISS)-MRA compared to CE-CTA and to Cartesian balanced steady-state free precession (bSSFP)-MRA. Prospective. Thirty patients with confirmed APE and 30 healthy volunteers (HVs). Radial QISS- and bSSFP-MRA at 1.5T. Signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR) were computed to compare the pulmonary imaging quality between MRA methods. The pulmonary arterial tree was divided into 25 branches and an ordinal scoring system was used to assess the image quality of each pulmonary branch. The clinical performance of the two MRA techniques in accurately assessing APE was evaluated with respect to CE-CTA as the clinical reference standard. Wilcoxon signed-rank and Spearman's correlation tests were performed. Sensitivity and specificity of the MRA techniques were determined using CE-CTA as the clinical reference standard. Thrombus-mimicking artifacts appeared more frequently in lobar and peripheral arteries of patients with Cartesian bSSFP than with radial QISS-MRA (pulmonary trunk: 12.2% vs. 14.0%, P = 0.64; lobar arteries: 35.6% vs. 22.0%, P = 0.005, peripheral arteries: 74.4% vs. 49.0%, P < 0.001). The relative increases in SNR and of CNR provided by radial QISS-MRA with respect to Cartesian bSSFP-MRA were 30-35% (P-values of SNR/CNR, HVs: 0.09/0.09, patients: 0.03/0.02). The image quality of pulmonary arterial branches was considered good to excellent in 77.2% of patients with radial QISS-MRA and in 43.2% with Cartesian bSSFP-MRA (P < 0.0001). The clinical performance of radial QISS-MRA was higher than Cartesian bSSFP-MRA for grading embolism, with a total sensitivity of 86.0% vs. 80.6% and a specificity of 93.3% vs. 84.0%, respectively. Radial QISS-MRA is a reliable and safe non-CE angiographic technique with promising clinical potential compared to Cartesian bSSFP-MRA and as an alternative technique to CE-CTA for the diagnosis of APE. 1 TECHNICAL EFFICACY STAGE: 3.

[Feasibility of Non-contrast-enhanced Coronary Magnetic Resonance Angiography at 3.0T].

Objective To evaluate the feasibility of non-contrast-enhanced magnetic resonance angiography (NCE-MRA) on a 3.0T scanner. Methods Totally 36 volunteers and 24 patients with clinically suspected coronary artery disease underwent NCE-MRA. The quality of the NCE-MRA images was graded for each segment on a four-point scale. The subjects were divided into two groups according to image quality. The age,body mass index (BMI),heart rate,end-expiratory diaphragm displacement,and respiratory diaphragm motion amplitude were evaluated and compared. Results The average image quality score of every segment was above 2 points. The proximal and middle segments of left anterior descending artery had significantly higher quality scores than the distal segments (P=0.000) and the proximal segment of left circumflex coronary artery had significantly higher quality scores than the distal segments (P=0.000),the proximal segment of right coronary artery also had a significant higher quality score than its distal segment (P=0.001). The image quality was good in 38 subjects (64.4%). The heart rate [(66.35±9.39) beat/min vs. (75.32±11.67) beat/min] (P=0.002) and the body mass index [(24.72±3.33) kg/m 2 vs. (27.82±3.61) kg/m 2] (P=0.002) were significantly different between the good image quality group and the poor image quality group. The end-expiratory diaphragm displacement in good image quality group was (4.43±2.07)mm,which was significantly lower than that in poor image quality group [(9.26±7.62)mm](P=0.013). The respiratory diaphragm motion amplitude [(21.35±6.02) mm] in good image quality group was significantly lower than that in poor image quality group [(30.68±14.20)mm](P=0.012). Conclusion NCE-MRA on 3.0T is a feasible tool for visualization of the proximal and middle segments of coronary arteries,and the image quality can be optimized by controlling heart rate and respiration in the future.

Intracranial simultaneous noncontrast angiography and intraplaque hemorrhage (SNAP) MRA: Analyzation, optimization, and extension for dynamic MRA.

Simultaneous noncontrast angiography and intraplaque (SNAP) imaging, as a noncontrast-enhanced MRA technique, may not provide consistent vessel visualization for intracranial artery imaging among subjects. This study aims to investigate the underlying mechanism and extend SNAP to dynamic MRA. The cause of the instability of intracranial SNAP-MRA was investigated through theoretical analysis and simulations. The scan parameters, including TI and flip angle, were optimized for reliable imaging. In vivo experiments were conducted to validate the simulation results. The simulation results were correlated with real intracranial blood flow by introducing the concept of blood travel time, and intracranial SNAP-MRA was revealed to reflect the cerebral blood expanse region in 5 TI. A new noncontrast-enhanced dynamic MRA technique, termed 4D SNAP-MRA, was proposed and demonstrated through in vivo scans. The cause of the instability of intracranial SNAP-MRA was proved to be the slow or fast blood flow in the imaging slab. This instability can be mitigated by adjusting TI and flip angle in the SNAP sequence. The proposed 4D SNAP-MRA can provide dynamic visualization of the cerebral blood circulation and cerebral hemodynamic information such as blood travel time. The 3D SNAP-MRA with optimal imaging parameters can generate cerebral angiography with hemodynamic information, and the 4D SNAP-MRA provides dynamic visualization of the cerebral blood circulation.

Relationship Between Blood Flow Velocity and Blood Suppression Inversion Time for Optimal Visualization of Renal Artery in Spatial Labeling with Multiple Inversion Pulses: In Vitro Study

Background: Improving imaging quality of non-contrast enhanced magnetic resonance angiography (NCE-MRA) is essential to those renal function impairment patients especially patients with renal segmental branch disease in the renal parenchyma. Objectives: To investigate the relation between blood flow velocity (BFV) and blood suppression inversion time (BSP TI) for optimal visualization of the renal artery in NCE-MRA by spatial labeling with multiple inversion pulses (SLEEK) sequence. Patients and Methods: A catheter (simulated renal artery) was used to perform SLEEK with various BSP TI when injected saline with different velocity (simulated BFV). The relationship between BFV and BSP TI was evaluated by measuring signal-to-noise ratio (SNR) and was confirmed in nine hypertensive patients. Results: A negative relationship was found in vitro study between BFV and BSP TI with SLEEK sequence (r = -0.977, P = 0.023). For BSP TI = 200 ms, the simulated renal artery displayed hyper-intensity when simulated BFV = 96 - 128 cm/s and the maximum SNR was found when BFV = 128 cm/s (SNR = 0.927). For BSP TI = 500 ms, high signal appeared with BFV = 64 - 128 cm/s and the greatest SNR was found with BFV = 96 cm/s (SNR = 1.732). For BSP TI = 800 ms and 1100 ms, hyper-intensity arose with BFV = 16 - 128 cm/s and BFV = 6.4 - 128 cm/s separately, the highest SNR was described when BFV = 32 cm/s (SNR = 2.256) and BFV = 16 cm/s (SNR = 1.875). The different optimal BSP TI with best image quality was discovered in nine hypertensive patients. Conclusion: An optimal BSP TI based on BFV can improve the ability of delineating renal artery in SLEEK sequence.

Time-of-flight MR-angiography with a helical trajectory and slice-super-resolution reconstruction.

To improve 2D noncontrast-enhanced MRA by using a helical time-of-flight (TOF) acquisition technique and a slice-super-resolution reconstruction. The TOF technique is combined with a helical trajectory with golden-angle-based radial projection reordering. A continuous spatial shift in slice direction is realized by adjusting the frequency of the excitation pulse between the individual projections. The limited resolution along the shift direction is improved by a deconvolution with simulated slice profile. The helical TOF (hTOF) was compared in vivo with a conventional 2D and 3D TOF. Results from in vivo experiments on the carotid show that the visual resolution in slice direction can be improved by using hTOF and the slice-super-resolution reconstruction. The vessels appear up to 1.5 times sharper and can be better separated from each other. Compared to 2D TOF images, the stair step artifacts are strongly reduced in reformatted hTOF images, whereas measurement time is decreased by at least 35%. Compared to 3D TOF, the hTOF offers a higher blood-to-background contrast, better visualization of smaller vessels, and reduced measurement time. The hTOF benefits from a 2D acquisition and a 3D reconstruction, which makes it a promising technique for the noncontrast-enhanced imaging of the carotid.

Evaluation of velocity-sensitized and acceleration-sensitized NCE-MRA for below-knee peripheral arterial disease.

To evaluate the diagnostic performance of velocity- and acceleration-sensitized noncontrast-enhanced magnetic resonance angiography (NCE-MRA) of the infrageniculate arteries using contrast-enhanced MRA (CE-MRA) as a reference standard. Twenty-four patients with symptoms of peripheral arterial disease were recruited. Each patient's infrageniculate arterial tree was examined using a velocity-dependent flow-sensitized dephasing (VEL-FSD) technique, an acceleration-dependent (ACC-FSD) technique, and our conventional CE-MRA technique performed at 1.5T. The images were independently reviewed by two experienced vascular radiologists, who evaluated each vessel segment to assess visibility, diagnostic confidence, venous contamination, and detection of pathology. In all, 432 segments were evaluated by each of the three techniques by each reader in total. Overall diagnostic confidence was rated as moderate or high in 98.5% of segments with CE-MRA, 92.1% with VEL-FSD, and 79.9% with ACC-FSD. No venous contamination was seen in 96% of segments with CE-MRA, 72.2% with VEL-FSD, and 85.8% with ACC-FSD. Per-segment, per-limb, and per-patient sensitivities for detecting significant stenotic disease were 63.4%, 73%, and 92%, respectively, for ACC-FSD, and 65.3%, 87.2%, and 96% for VEL-FSD, and as such no significant statistical change was detected using McNemar's chi-squared test with P-values of 1.00, 0.13, and 0.77 obtained, respectively. Flow-dependent NCE-MRA techniques may have a role to play in evaluation of patients with peripheral vascular disease. Increased sensitivity of a velocity-based technique compared to an acceleration-based technique comes at the expense of greater venous contamination. 2J. Technical Efficacy: Stage 2 J. MAGN. RESON. Additive value of non-contrast MRA in the preoperative evaluation of potential liver donors

The purpose of this study is to compare diagnostic quality, inter-observer variability and agreement of non-contrast enhanced MRA (NC-MRA) with contrast-enhanced MRA (CE-MRA) in the evaluation of hepatic arterial anatomy. 20 potential liver donors were included in this retrospective study. NC-MRA, CE-MRA and combined data sets were randomized and reviewed by two readers. Reference standard was consensus by two senior radiologists using all data including CTA. There was no difference in IQ or diagnostic confidence between NC-MRA, CE-MRA or combined data for either reader but the arterial origin of segment IV was successfully identified on NC-MRA when CE-MRA was suboptimal.

Preoperative evaluation of renal artery in patients with renal tumor

To investigate the feasibility of the noncontrast-enhanced magnetic resonance angiography (NCE-MRA) to evaluate renal arteries before partial nephrectomy (PN).Retrospective analyzed 479 patients who underwent renal surgery between January 2013 and December 2015 with NCE-MRA or computed tomographic angiography (CTA) renal artery image reconstruction preoperative in our department. The renal artery reconstruction score (RARS) was based on the level of artery visualization in a 4-class criterion, and the R.E.N.A.L nephrometry score (R.E.N.A.L), arterial based complexity (ABC) were also analyzed.Of the 479 patients, the overall-lever RARS was 3.62, and the average in 2 groups was no significant difference (NCE-MRA vs CTA, P = 0.072). The performance of NCE-MRA in PN group was similar with CTA. Further comparison demonstrated that the efficiency of NCE-MRA in moderate- or low-degree tumor according to the R.E.N.A.L and ABC complexity less than 3S was equal to CTA. However, high degree (P < 0.001), 3S (P = 0.027), or 3H (P < 0.001) would affect the imaging of renal artery. Intragroup analysis showed that tumor complexity such as max tumor size (r = −o.351, P < 0.001), R.E.N.A.L (r = −0.439, P < 0.001), and ABC (r = −0.619, P < 0.001) were closely correlated with the NCE-MRA performance. The images of 2 sides of the kidney were compared in single person as well, which was meaningful for NCE-MRA patients only (NCE-MRA, P < 0.001; CTA, P = 0.182).The renal artery reconstruction performed by NCE-MRA is feasible and has a similar achievement in the PN potential recipients, with a lower side effect, and meets the requirements for making surgical decision. It has a broad application prospect in clinical practice; however, it still needs to further improve the ability in more complex tumors.

Comparison of computed tomographic angiography and noncontrast magnetic resonance angiography in preoperative evaluation of living renal donors.

Introduction:The computed tomographic angiography (CTA) renal donor protocol is an established method of preoperative renal vascular pedicle evaluation in prospective renal donors. However, CTA is associated with significant radiation exposure and intravenous contrast administration. The newer noncontrast-enhanced magnetic resonance angiography (NCE-MRA) techniques, especially arterial spin labeling (ASL) with steady-state free precession (SSFP) hold promise as an effective alternative. We prospectively compared CTA with NCE MRA for accuracy in the evaluation of renal arterial anatomy in prospective renal donors.Methods:A total of 43 subjects underwent CTA followed by NCE MRA in a prospective comparative study. The number of renal arteries and early branching of renal arteries were noted in both kidneys in all subjects. Intermodality agreement was calculated using “K” (Kappa) statistics and 95% confidence interval for both modalities.Results:A total of 63 single, 21 double, and 2 triple arteries were detected in 43 subjects on CTA. CTA showed an early branch in 17 kidneys. NCE MRAshowed 64 single arteries, 20 double arteries, and 2 triple arteries. A total of 14 kidneys showed an early branch. Unweighted Kappa statistic of agreement between CTA and NCE MRA for number of renal arteries and for frequency of early branching was 0.9707 and 0.8822, respectively.Conclusions:The newer NCE MRA techniques such as ASL with SSFP among others are potential alternatives for CTA, in the evaluation of prospective renal donors.

Noncontrast-Enhanced Magnetic Resonance Versus Computed Tomography Angiography in Preoperative Evaluation of Potential Living Renal Donors

Living renal donors undergo an extensive examination program. These examinations should be as safe, gentle, and patient friendly as possible. To compare computed tomography angiography (CTA) and an extensive magnetic resonance imaging (MRI) protocol without contrast agents to observations from nephrectomy in living renal donors and to evaluate whether noncontrast-enhanced MRI can replace CTA for vessel assessment in living renal donors. CTA and MRI results were compared to observations from nephrectomy, which served as the reference standard. Fifty-one potential kidney donors underwent imaging, and 31 donated a kidney. Comparisons in sensitivity, specificity, and accuracy were made with respect to the number of arteries, early branching, and the number of veins. Agreement was assessed using Cohen's kappa. The exact McNemar's test was used to test for statistically significant differences. In the assessment of more than one renal artery, the sensitivity and specificity of MRI and CTA were high and in perfect agreement compared to observations from surgery. The results for both MRI and CTA were as follows: (sensitivity 100%/specificity100%/accuracy 100%/Kappa = 1/P = 1). When comparing the ability to test for early branching we found, MRI: (sensitivity 33%/specificity 100%/accuracy 87%/Kappa = 0.45/P = 1) and CTA: (sensitivity 50%/specificity 100%/accuracy 90%/Kappa = 0.62/P = 1). When used to depict supernumerary veins, we found MRI: (sensitivity60%/specifivity100%/accuracy 93%/Kappa = 0.72/P = 1), whereas CTA showed: (sensitivity 40%/specificity 96%/accuracy 87% Kappa = 0.43/P = 1). In conclusion, an optimized MRI protocol that includes noncontrast-enhanced magnetic resonance angiography can be substituted for CTA for preoperative assessment of the renal vessels before living donor nephrectomy.

Non-contrast-enhanced MR angiography for detecting arteriovenous fistula dysfunction in haemodialysis patients

To assess the diagnostic value of non-contrast-enhanced magnetic resonance angiography (NCE-MRA), using time-of-flight and black-blood MRA, in the evaluation of arteriovenous fistulas in haemodialysis patients in comparison to multidetector computed tomography angiography (MDCTA). NCE-MRA and MDCTA were performed on the same day in 21 patients on maintenance haemodialysis with dysfunctional arteriovenous fistulas. The fistulas included three segments: arterial inflow, anastomosis, and venous outflow. Two experienced observers, who were blinded to the results of the NCE-MRA, recorded in consensus the significant stenoses (≥50%) seen on CTA. Two other experienced observers, unaware of the results of CTA, independently recorded significant stenoses (≥50%) in the NCE-MRA. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of NCE-MRA were calculated, with MDCTA as the standard reference. Sixty-three vascular segments in the 21 patients were clearly displayed. For the two observers of NCE-MRA, the accuracy was 98% and 95.4%; sensitivity 96.4% and 96.4%; specificity 97.1% and 94.3%; positive predictive value 96.4% and 93.1%; and, negative predictive value 97.1% and 97.1%. Inter-/intra-observer agreement for detecting stenosis was excellent for NCE-MRA, with a weighted kappa of 0.968 (95% confidence interval [CI], 0.874-1) and 0.936 (95% CI, 0.848-1). Non-contrast-enhanced MRA, using time-of-flight and black-blood MRA, is a reproducible and reliable imaging technique for detecting ≥50% stenosis in dysfunctional haemodialysis arteriovenous fistulas.

Improved artery–vein separation with acceleration‐dependent preparation for non–contrast‐enhanced magnetic resonance angiography

To compare the use of acceleration-dependent and velocity-dependent flow-preparation for non-contrast-enhanced magnetic resonance angiography (NCE-MRA), investigating both image quality and the ability to discriminate between arteries and veins. We develop an acceleration-dependent NCE-MRA method known as acceleration dependent vascular anatomy for non-contrast-enhanced MRA (ADVANCE-MRA). Acceleration-dependent and velocity-dependent images were acquired using a constant and pulsatile flow-phantom and from the lower legs of six healthy volunteers and one patient with peripheral vascular disease. The volunteer images were assessed both by quantitative signal measurements and qualitative scoring by a radiologist. In the phantom, acceleration-dependent preparation depicted pulsatile but not constant flow, while velocity-dependent preparation depicted both. In the volunteers and the patient, the velocity-dependent preparation was unable to separate the arterial and venous signals completely, with some overlap of arterial and venous signals for all acquired flow sensitizations whereas the acceleration-dependent preparation gave complete artery-vein separation over a wide range of flow sensitizations. Acceleration-dependent preparation received the best overall qualitative scores for arterial image quality and venous contamination. Acceleration-dependent NCE-MRA improves arterial image quality and reduces venous contamination, compared with velocity-dependent NCE-MRA, and warrants further investigation in patients.

Non-enhanced MR angiography of renal arteries: comparison with contrast-enhanced MR angiography

The main causes of renal artery stenosis (RAS) are atherosclerosis and fibromuscular dysplasia. Despite contrast-enhanced magnetic resonance angiography (CE-MRA) being a safe and reliable method for diagnosis of RAS especially in young individuals, recently it has been possible to adopt innovative technologies that do not require paramagnetic contrast agents. To assess the accuracy of steady-state free-precession (SSFP) non-contrast-enhanced magnetic resonance angiography (NC-MRA) by using a 1.5 T MR scanner for the detection of renal artery stenosis, in comparison with breath-hold CE-MRA as the reference standard. Sixty-three patients (33 men, 30 women) with suspected renovascular hypertension (RVHT) were examined by a 1.5T MR scanner; NC-MRA with an electrocardiography (ECG)-gated SSFP sequence was performed in 58.7% (37/63) of patients; in 41.3% (26/63) of patients a respiratory trigger was used in addition to cardiac gating. CE-MRA, with a three-dimensional gradient echo (3D-GRE) T1-weighted sequence, was performed in all patients within the same session. Maximum intensity projection (MIP) image quality, number of renal arteries, and the presence of stenosis were assessed by two observers (independently for NC-MRA and together for CE-MRA). The agreement between NC-MRA and CE-MRA as well as the inter-observer reproducibility were calculated with Bland-Altman plots. MIP image quality was considered better for NC-MRA. NC-MRA identified 143 of 144 (99.3%) arteries detected by CE-MRA (an accessory artery was not identified). Fourteen stenoses were detected by CE-MRA (11 atherosclerotic, 3 dysplastic) with four of 14 (28.5%) significant stenosis. Bland-Altman plot demonstrated an excellent concordance between NC-MRA and CE-MRA; particularly, the reader A evaluated correctly all investigated arteries, while over-estimation of two stenoses occurred for reader B. Regarding NC-MRA, inter-observer agreement was excellent. NC-MRA is a valid alternative to CE-MRA for the assessment of renal arteries.
